Duke reimbursements
Below is information on paperwork needed to be reimbursed through
Duke's NSF grant.
For all reimbursees:
Please fill out the
reimbursement form.
- Please provide your private/home mailing address (Duke will not
mail reimbursement checks to institutional addresses) and your
email address in case of follow-up questions.
- Please provide your social security number (equivalently,
taxpayer identification number)
- Please provide all receipts for your expenses:
- Airline ticket (need original)
- Boarding passes (if posible)
- Hotel (if not directly billed to the university)
- Any other travel receipts (car rental, gas, taxi, parking)
- Actual meal receipts: Duke does not pay a domestic per diem rate.
- Fill out Duke's
certificate
of foreign status
- Fill out a declaration
form
For foreign nationals:
There is, unfortunately, additional paperwork.
All foreign nationals should provide
- A copy of an invitation letter/email
- A copy of your passport main page
- A copy of the visa page
- A copy of the front and back of I-94 form
- B2, WT status: a filled out declaration form
- F1 visa:
- a copy of the I-20 form
- Students: a letter from your home institution's ``Visa Services'' (or
equivalent) department stating that you are a current
student and are in good standing, ideally requested before the date of the
meeting.
- J1 visa: copy of DS-2019 form
- Fill out IRS form 8233
if eligible for tax treaty
Mail all forms, copies, receipts, and address information to

Please mail all this so that it will be received not later than one
month after the date of the regional meeting. Receipts etc received
after that date might not be reimbursed.
